Our insurance requires that all guests, including divers and non-divers, complete an RSTC Medical Form prior to boarding Aqua Cat.
To expedite your check-in process onboard, we are asking that all guests sign our Liability Release, Shark Waiver, and Medical Questionnaire via WaiverFile. Make sure you complete the trip application after signing the required pre-trip documents.

Please read through the Medical Form when digitally signing to confirm whether you will need for it to be signed off by a physician. If on your medical you answered YES to questions 3, 5, 10, OR to any of the questions on page 2, please take all three pages of this form (Participant Questionnaire and the Physician’s Evaluation Form) to your physician for a medical evaluation. Participation in diving will require a Physician’s approval. Failure to properly complete the Medical Form can result in not being allowed to participate in diving or snorkeling.

All travelers*** to Cuba need a passport, valid for at least six months after the date of entry. Check with the Cuban Embassy for visa if citizenship is not U.S.

Visas are issued at your home (U.S.) airport at time of airport check-in.
PASSPORTS & VISAS - A passport and a visa are required to enter Egypt. The passport must be valid for at least 6 months from date of entry into Egypt. A visa can be purchased at the Egyptian airport for $25 USD. Non-U.S. citizens should check with the appropriate consulate regarding passports and visas.

For Cuba dive cruises, the transfer bus from Havana to Port Jucaro, where the All Star Avalon II is docked, only picks up passengers at two Havana hotels - Hotel Iberostar Parque Central and Melia Habana - Miramar Oceanfront Hotel, shown here.
Guests staying at any other location will need to take a taxi to one of these hotels for their transfer to the vessel. Please arrive well in advance to not miss the bus.
Pickups from Havana are in the early morning (approximately 8:00am) and arrive at the vessel by mid-to-late afternoon (3:30-5:30pm). This trip from Havana to the dock is 6-7 hours long and the cost is approximately $100.00, depending on the number of passengers.

U.S. citizens are prohibited from engaging in financial transactions with those on the list. If you choose not stay at one of the two recommended hotels above, please reference this list before booking a hotel or shopping in Old Havana.
